How often should you update your passwords, and what makes a password truly secure?

How often are you actually changing your passwords and do you really know what makes them secure? Most security experts recommend updating passwords every 90 days but honestly having a super strong password with two-factor authentication might be more important than constantly changing weak ones. Are you using unique passwords for each account or falling into that dangerous habit of reusing the same one everywhere? What's your current strategy for keeping your accounts safe?
 
There is often a guide while you make the password. Note, however, that I make my own difficult passwords. I don't use recommendations cause they're harder to remember.
 
In my case, a strong password has to be long, random, and unique for every site I use. I use a password manager because remembering all of them is rather impossible. And I do change my password immediately I'm notified of any breach.
